(Reservations are valid until through 11:59pm on the 12th. The 13th-14th is an open apply period, meaning even if a character was reserved that month, you may apply for the character at that point unless an application has already been submitted. We do not accept application challenges.)

Applications will be submitted via a Google Form. At the start of the application, it has a required email address collection. This is for the purpose of sending out receipts with an automated system. If you do not want to provide your email address or do not want a receipt, please use isleofavalon.mods+no@gmail.com in this field to proceed.

- Name
- Contact Information (DW Journal or email address for us to contact you regarding your application)
- Characters Currently Played In-Game
- Term of Game Membership Agreement- the game rules are listed. You must agree to them in order to continue in the application. If you click "I disagree" the form will be submitted as-is and exit out of the application.
Basic Character Information:
- Name of Canon (If you are applying for an original character, type "Original Character")
- Character Name
- Age
- Canon Point - Our policy on spoilers is that your canon point or canon release date must be available to the public for at least 30 days. This has been our policy and has been listed in our FAQ since the start of the game. Failure to adhere may result in revisions (if adjustable canon point) or automatic rejection (if character or canon was introduced less than a month ago).
- Canon Information (link to a wiki- if you do not have anything to link to or are applying for an original character, you may provide a brief information section of your own. 1200 words or less. Do not link to the entire canon.)
- We typically just want a summary of canon events as pertains to your character so we have something to go off of in terms of context while we're researching your character for application processing. Backstory and personality information integrated would be great if a wiki is not available. Bullet points are fine.
- CRAU Designation - If you say no, the following section (CRAU Information) will be skipped. Go straight to "In-Game Information."
- What game are you importing your character from?
- Provide a brief summary of developments your character has experienced at other DWRP games.
- Familiar Choice (include top choice only unless you want help deciding from moderator. Please reference the FAQ for more information about familiar and magic.)
- Primary Magic Choice (Please reference the FAQ for more information about familiar and magic.)

- Inventory (5 personal items) - Canon/CRAU-unique items are required to have either a link to an item description or a brief list of the item's capabilities.
Play:
- Thread Sample (TDM, Meme- do not link smut. Must have at least 5 consecutive comments from your character. Threads from another game are also acceptable as long as they provide a good sample of character voice and are accessible to non-members. Thread should be no older than 6 months.) *Note: If you were dropped via activity check, you are required to provide a thread from the current test drive meme. See FAQ for more details.
- Answer your choice of 5 out of 20 listed questions as your character (bracket or prose are fine but your character is responding, you the player are not responding about your character). If your character would refuse to answer, have them respond how they would naturally, but include internal dialogue in either prose or brackets (your choice) with their thoughts on it. Questions are chooseable from a drop-down menu in this section of the app, so you can pick ones that you find interesting or relevant to your character to respond to. If you are applying on mobile and cannot see the full length of the question, here is a list for your convenient access.
